Community nursing
Quality nursing care in the place you feel most comfortable, your own home, delivered by qualified, compassionate nurses.
Nursing care at home
At Rehoboth, we believe quality nursing care should be accessible in the place you feel most comfortable: your home. If you live with disability or have complex health needs, our community nursing services provide professional clinical support, claimable against your NDIS plan where eligible, to help you stay healthy, safe and independent at home.
We also provide peace of mind for families and carers, offering ongoing assistance and education so they can confidently manage their loved one's health needs.

Our approach
Every person's health journey is unique. That is why our care plans are flexible and tailored to each person's individual needs, routines and preferences. Our nurses work closely with participants, families and healthcare professionals to provide seamlessly coordinated care that upholds dignity, respect and empathy.
We also encourage participants to build confidence in managing their own health through practical education and supports that promote independence. If you need clinical support around the clock, our 24-hour care team can combine nursing with everyday supports, and our specialist services are available for therapy and allied health needs.
Funding under the NDIS
If you have high or complex health needs related to your disability, community nursing care may be funded under the Capacity Building category called Improved Daily Living in your NDIS plan. Rehoboth is a registered NDIS provider, and our team can work with your support coordinator or plan manager to confirm eligibility and organise nursing supports that suit your needs.
You can also explore your funding options or learn more about how the NDIS works.
