Everything you need to start services with Rehoboth, and a friendly team who will complete each form with you, step by step.
Client intake form
Our client intake form is the starting point for new participants. It gives our team a clear picture of who you are and the supports you need, so we can match you with the right people from day one. The form captures:
- NDIS details, your NDIS number, your plan type (agency managed, plan managed or self managed) and your plan end date
- Supports required, such as a support worker, social and community participation, allied health assistance, mental health mentoring, occupational therapy, recovery coaching, nursing care, physiotherapy or domestic care
- Mobility and personal care needs, including any assistance required with showering, toileting, grooming, dressing or eating, plus communication preferences and your current living situation
We don't ask you to fill this out alone. To complete your intake, simply call us on 1300 853 095 or email info@rehoboth.com.au and our team will walk you through every question over the phone or by email. We will also let you know which documents are helpful to share, such as your NDIS plan or any recent assessment reports. Referral form
Are you a support coordinator, health professional or family member referring someone to Rehoboth? Our referral form gathers the participant's contact details, funding type and the services being requested, from support workers and community participation to supported accommodation, nursing care, physiotherapy and occupational therapy.

What happens next
Once your intake or referral is with us, here's what you can expect:
- We review your information, a member of our team reads through your details and any documents you've shared, and contacts you if anything needs clarifying
- We arrange a consultation, we sit down with you, and anyone you'd like involved, to talk through your goals, routines and preferences
- We build your support plan, together we shape supports around your NDIS plan, agree on a schedule and introduce you to your support team
