Making a referral to Rehoboth is simple, whether you're referring yourself, a family member or someone you support, we'll take it from here.
Who can make a referral
Anyone can refer a person to Rehoboth. We welcome referrals from across VIC, NSW and QLD, including:
- Self-referrals, if you're an NDIS participant looking for the right supports, you can contact us directly. There's no need to go through anyone else.
- Family members and carers, parents, partners, siblings and informal carers who want to find quality support for someone they love.
- Support coordinators and plan managers, we work closely with coordinators to match participants with supports that fit their plan and goals.
- Hospital discharge planners and social workers, we can help arrange in-home support, nursing care or accommodation so a discharge isn't delayed.
- Other providers, if a participant needs a service you don't offer, we're happy to work alongside you.

What we need from you
To get started, we just need a few details about the person being referred:
- Participant's full name and date of birth
- Contact details, phone, email and address
- NDIS number and plan type (agency-managed, plan-managed or self-managed)
- Primary contact person and their relationship to the participant
- The services required, for example personal care, community participation, supported independent living (SIL), short or medium term accommodation, or nursing care

What happens next
Once we receive your referral, here's what you can expect:
1. We respond promptly
A member of our team will be in touch to confirm we've received the referral and answer any initial questions.
2. Free, no-obligation consultation
We'll arrange a time to chat with the participant and their family or supporters, at home, over the phone or at a place that suits them. We listen first, so the supports we propose genuinely reflect the person's goals and preferences.
3. Service agreement and getting started
If everyone is happy to proceed, we prepare a clear service agreement, match the participant with the right support workers and agree on a start date. We'll also stay in touch with the referrer where appropriate, so everyone is on the same page.
